Check if your patient is eligible
Use our Find a Test Directory to search by condition, gene or R number and it will tell you what the eligibility criteria is for each test. You will need to explicitly explain how your patient meets the eligibility criteria.
Genetic testing may occasionally be appropriate outside these criteria following discussion at a specialist MDT with a geneticist present.

For help to complete the form, GeNotes has some useful information. You’ll need to complete the form electronically as we don’t accept handwritten forms. One Test Order Form per trio.
You must include on the form how your patient meets the eligibility criteria.
Include the patient’s ethnicity to improve accuracy of results. We need to compare results with population specific data to understand if variant could be causative
Don’t forget to include your HPO terms on the order form which start with the terms HP. Even those that might not seem relevant, otherwise we might miss relevant findings. Find them hereÂ

We require two samples : one somatic sample from the tumour as fresh frozen tissue and a germline blood sample in an EDTA tube.
DNA will be damaged if it’s not stored in the correct format.
Have you organised a blood sample? If the patient is organising a local blood test, they will need a specific blood form. Please complete all the patient details and demographics before giving the form to a patient.

We’ll email you if there is a problem. Testing will be cancelled after 16 weeks if the referral isn’t complete

Samples & requirements
For cancer testing, we require two samples : one somatic sample from the tumour as fresh frozen tissue and a germline blood sample in an EDTA tube.
Where clinically appropriate (such as haem-onc cancers) and if no fresh or stored blood or fibroblast derived DNA is available, DNA extracted from saliva samples may be used.
